At The Medical Center of Aurora we realize how important it is for patients to obtain copies of medical records for both following up with physicians after being hospitalized, and for personal record keeping.

We offer two ways to access your medical record:

MyHealthOne Portal:

MyHealthOne consolidates many common tasks into one secure, easy-to-use online patient portal. It gives you access to most of your medical records on your desktop computer, laptop, tablet or smartphone 24 hours a day.

*Note that some medical records may only be available through our hospital Medical Records office.

Paper Copy of Medical Records:

As of May 1, 2016, all record requests must go through the HealthONE Centralized Release of Information Center. As such, medical records will no longer be available for pick-up at the facility. To request your medical records you will need to complete an Authorization form, which can be downloaded via below links. Once you have completed the form, you may fax it to the number on the bottom of the form or mail it to:

Birth and Death Certificates

Certified birth and death certificates are processed and purchased through The Colorado Department of Health, Vital Statistics department.

Frequently Asked Questions

How soon will I receive my records?

Requests are processed 7-10 business days from the receipt of your request.

What is the cost for my record?

$0.25 per page for paper records and $0.02 per page for electronic records.

How do I have my records sent to another physician?

You may designate that your records be faxed by your appointment date to your physician’s office on your release of information. Please include your physician’s name and fax number.

Your physician may request your records to be faxed directly to their office for continuity of care purposes. Affiliated physicians may access your medical records via our Electronic Medical Records for all visits prior to 6/18/2012.

Where can I obtain an Authorization form to request my medical records?

Authorization forms can be picked up in the Health Information Management Department or can be printed from our website at http://auroramed.com/patients/medical-records.dot.
